Florence Nightingale – A Name of its Own

Florence Nightingale was born into privilege and wealth. Born in Florence, Italy on May 12, 1820, she received her name from the city of her birth. It is noteworthy that Florence was considered an unusual name at the time, but this changed with the fame of Miss Nightingale. Although born in Italy, she grew up in Derbyshire, Hampshire, and London, where her parents kept large, comfortable homes. Nightingale was the offspring of parents of upper-class power and influence, She was the younger of their two daughters and her upbringing was a classical education and social prominence. Miss Nightingale†s classic education included history, multiple languages, economics, astronomy, science, philosophy, and mathematical skills. Mr. Nightingale took upon himself the classical education of Florence while Mrs. Nightingale taught her the social skills and leadership qualities. Florence became increasingly unhappy with her upper-class lifestyle. Victorian England afforded women of upper-class means to a life of little education, but an emphasis on proper male-female roles. A woman†s role in 19th century England was to be subservient to her husband and avoid occupational ambitions. The Nightingale family ignored the custom of offering little education to their girls. On February 7, 1837, Florence said she heard the voice of God tell her that she had a mission. Nine years later she discovered her mission. In 1846, a friend sent her the Year Book of the Institute of Protestant Deaconesses at Kaisersworth, Germany, which is a training school for nurses. Four years later Florence left for Germany to attend the institute during 1850, and again returning to Germany in 1851. These two brief periods were the only formalized medical training she received. Miss Nightingale†s mother never supported her nursing desires, and Mr. Nightingale opposed her nursing aspirations, but gave his blessing after becoming ill and receiving attentive care from his nursing student daughter. In 1853, Nightingale received an unpaid position of Superintendent of Nurses at the Institution for the Care of Sick Gentlewomen in Distressed Circumstances in London. This position allowed her to display innovative skill for nursing and nursing leadership. She established numerous changes in nursing care which included hot and cold running water on all floors, and patient call lights. In 1854, Miss Nightingale was called upon during the Crimean War. Nightingale assembled 38 women on October 21, 1854, and left for Turkey two weeks later to care for the injured and diseased British solders. For 21 months, Nightingale†s team of women established hygienic standards in the care of the wounded, and established a pure water supply. When she arrived more men were dying from fever and infection than the war wounds themselves. She enforced sanitary regulations, and introduced special diets. With her own money she bought she bought linen, shirts, food, and even beds for the military hospital. As a result of these efforts, the mortality rate dropped to 2%, down from 60% previous to their arrival. In 1860 the Nightingale School, a formal nursing training program opened with only 10 students. Although nursing education was not her utmost personal mission after returning from the war, it soon became her mission. Florence Nightingale was a pioneer and a brilliant motivator and organizer. She is credited with establishing nursing as the respected profession it is today. Florence Nightingale is responsible for not only modern western nursing education, but also secularized nursing training. Although a believer in a perfect and eternal God who developed laws that ruled the universe, she came to believe that all people, without regards to religious affiliation or belief, are entitled to medical and nursing care. Her influence extended beyond mere buildings and institutions. Her mission was guided by a prevailing philosophy of nursing springing from past experiences in Crimea and London. Her conceptual theories were later refined and improved upon, as other great theories have developed over time. But the rich legacy she left behind has to do with ideas. Among these is the nursing model that treats patients as multidimensional individuals rather than merely treating the disease. Nightingale popularized the view that the purpose of nursing was to â€Å"put the patient in the best possible condition for nature to act upon him†(Nightingale, 1859). This is now known as the environmental adaptation theory. Nightingale was also a world known writer. Notes on Nursing was her most famous book in which was written as a guide for the general public as an outline for various hygienic and sanitary improvements. Her main concern was how the public could alter the environment in order to increase health and wellness of patients. The lifelong works of Florence Nightingale have made the nursing profession what it is today and has greatly influenced the patient care ethics of Western civilizations.

Dow was now unwilling to pay the $15. 3 billion for Rohm and Haas and consequently tried to delay the process. With this situation, the dilemma of DOW was perfect, a global economic meltdown and a promise which DOW was not able to finish because of a deal break. The whole economy changed after the pronouncement in July that the second largest chemical company would like to by Rohm and Haas. Stock prices of chemical companies increased immense and the shareholder of Rohm and Haas expect a large profit from their shares especially in the ongoing global downturn, pressing the stock values to the bare bone. Rohm and Haas forced DOW to finish the promised acquisition but DOW never got a specific plan how to handle this complex and significantly changed situation. Just an email from the chairman of DOW explained that it would be better to wait till June 2009 in order to finish the deal. After they heard that Dow invested in different other projects it was simply too much for Rohm and Haas, as they could not understand why other deals were made but the acquisition was still delayed, so they started a litigation to enforce the merger. Rohm and Haas and DOW conducted several meeting to find a way to resolve the situation, but without any success and to the great disappointment of Rohm and Haas as their expectations were not met at all. American Colonies

The American Revolution can be considered as the most important event within American history. Through this event, a collection of colonies transformed themselves into a unified government and society, one that embarked on the longest and most successful demonstration of democracy in world history. During the 18th century, England and France were embroiled in a series of wars that were ultimately meant to establish one or the other as the dominant European force. The first three wars had no true impact on the colonies, but the last war – the French and Indian War – would lead to a huge change in the relationship between the colonies and England. During the early years of the war, prior to 1758, the colonists continued trading with the French while refusing to contribute money to the British war effort. The first problem was solved through the efforts of the William Pitt, Prime Minister of England. He offered to reimburse the latter for part of any war expenses they incurred on behalf of England. The second issue resolved itself through victories the English had in Canada and the Caribbean. Unfortunately, the English did not realize that their war policies would end up putting their realm in severe debt. Once it was realized, however, it opened the door for reform programs that would ultimately push the American colonies to seek independence from England. Following William Pitt as Prime Minister were several men, all of whom tried to enforce strict laws on the American colonies with the goals of replenishing the English treasury and reestablishing English authority over the colonies. The first of these men was George Grenville, who became Prime Minister following William Pitt. Grenville assessed the situation in the following manner: the colonies had an extremely light tax burden as compared to that being endured by the English; therefore, it would be logical for the prosperous colonies to share in the expenses that had been racked up trying to defend and protect them. Grenville further discovered how lax the royal customs service in the American colonies was. This, combined with the need for England to replenish its treasury, lead to the implementation of several acts that would meet with ever increasing anger and protest on the part of the American colonists. First, Grenville saw to it that the Navigation Acts that had been ignored for so long were now implemented to the last letter. Next was the passage of the Sugar Act in 1764, which put new taxes on foreign imports such as wine, textiles, coffee, indigo, and sugar. The goal behind this particular tax was to bring in enough money to offset the expenses of â€Å"defending, protecting and securing† (Shi Tindall, 2007) the colonies. Following the Sugar Act was the Currency Act of 1764, which put a stop to the colonies printing their own paper money. Following the implementation of the Sugar Act was the Stamp Act of 1765. This act stated that all printed material within the colonies had to had a revenue stamp on them. The final act implemented by Grenville was the Quartering Act, which applied to all colonies, but most seriously affected New York. It required that soldiers of the British army be given lodging within the homes of any colonial family at any given time. In the minds of the colonists, all these various acts infringed on their rights. They strongly believed that England had no right to tax them if the colonists had no representation within the English Parliament. Therefore, each act that was passed was met with great protest and outrage within the colonies. Eventually, the slogan that could be heard throughout the colonies was: â€Å"No taxation without representation† (Shi Tindall, 2007). The outrage over the various acts, particularly the Sugar and Stamp Acts, ultimately led to the downfall of Grenville, and the repeal of these two offensive acts. However, Grenville’s successor went down the same path in passing the Townshend Acts. These laws were intended to bring the colonies, particularly New York, to heel. Instead, they simply increased colonial resentment and resistance. Yet, the level of resentment and resistance varied among the colonists. John Adams stated during the first Continental Congress that, with regard to where the colonists stood on independence, one third were for independence, one third were against independence, and the final third were undecided. Adams was extremely accurate in this statement. From the start, there had been a select group of colonists that agitated for immediate independence in response to the way Parliament and England treated them. Just as adamant were a group who believed that the colonies owed everything to England, and therefore, could not see breaking away from England as a good thing. Finally, there was the group who could not decide either way. Like all undecided groups, they could see valid points on each side of the issue, but were unable to make a definitive choice. Ultimately, one of the deciding factors that rallied all the colonists to approve gaining independence from England was Thomas Paine’s Common Sense. In this pamplet, Paine outlined points that the majority of the colonists agreed with. He did not just attack Parliament, but also the concept of monarchy. For Paine, the responsibility for the troubles in the colonies did not just lay at the feet of Parliament, but also at the feet of King George III himself. Paine used his pamphlet to present the following conclusion: â€Å"Americans should consult their own interests, abandon George III, and declare their independence† (Shi Tindall, 2007). Another aspect that unified the majority of American colonists in seeking independence was the outbreak of war in Lexington and Concord, Massachusetts. These two battles occurred before a formal break with England had been pronounced. The fact that English soldiers would attempt to initiate warfare with the colonists before they had even completely decided on breaking away from England was more than enough evidence that England did not have any consideration for the colonial point of view. Therefore, the formal break England thought would never occur did occur via the writing and proclaiming of the Declaration of Independence in July of 1776. Overall, the American Revolution occurred for one reason: it was the right time and moment to establish country that ruled in a manner completely different from the norm of monarchy. Many of those watching the Revolution unfold believed it was nothing more than an experiment in the misguided concept of democracy that would ultimately fail. Yet, that it did not fail is a testament to the hard work of and constant discussing and debating of the issues by the colonists. They took what they felt was best from monarchical government and adapted it to fit into a new, democratic form of government and society, one in which all inhabitants of that society would have a say in the governing of that society. The only result is that the men regard her as a slut, and Curley becomes even more intensely jealous. Finally, her loneliness leads to her death as she makes the mistake of trying to defeat it by playing with Lennie. Curley himself is lonely. His new wife hates him, as do all the ranch hands who dislike him for his cowardice. He has married, to try and escape his loneliness, but has chosen a wife totally inappropriate for the kind of life he desires. His feelings are all of aggressive behaviour, which places his wife further away from him and leads to the incident with Lennie where his hand is crushed. Crooks is another who is isolated because he is different. He copes with it by keeping a distance between himself and the other hands. When he does allow himself to be drawn into the dream of working on George and Lennies dream farm, he is immediately shut out by Georges anger. * Violence The story has many examples of a kind of needless violence. For example, Candy relates how the boss gave them whisky and allowed a fight to take place in the bunkhouse. Curley is the most obvious violent character, and whenever he appears there is a feeling of tension. He causes George to remarkwhat the hells he got on his shoulder. Candy explains that Curley often picks on big guys (a sure sign of trouble for Lennie). We are prepared for Curleys later anger, which adds up at the end in his wish to shoot him in the guts. Carlson is another character associated with violence. He is unconcerned about killing Candys dog (and in fact cleans the gun in Candys presence). He goes to watch the fun when Curley thinks Slim may be with his wife, and later threatens Curley more, saying kick your head off. Later he is very keen to get his gun to join in the hunt for Lennie. The last words in the book belong to Carlson, and it is little surprise that they reveal his complete inability to understand Georges feelings about the death of Lennie. Compared to the other characters, Lennie reveals an unintentional violence. He does not even think to fight back when Curley attacks him, but when he does, it is with immense and uncontrollable power. He has so little control over his own strength that he accidentally kills his puppy, and then minutes later snuffs out the life of Curleys wife. His actions on these occasions are compared to those of an animal, powerful but thoughtless. Curleys wife is attracted to him because of the violence he had shown in crushing her husbands hand. It is the threat of violence to be used against Lennie that causes George to take the final step of killing his friend. * Dreams Dreams are one of the ways in which the characters fight the loneliness and hopelessness of their day to day lives. The most obvious example is the dream farm, a dream shared at first only by George and Lennie, but which later spreads to include Candy and Crooks. Crooks reveals that it is the favourite dream of the ranchers: Seems like ever guy got land in his head. It is a powerful dream, and even Crooks falls under its spell for a short time. To Lennie, the dream is a cure to disappointment and loneliness, and he often asks George to say the description of the farm to him again. Curleys wife is another who has dreams, her fantasies of a part in the movies and a life of luxury. Part of her sadness with her life is that it can never measure up to her dreams. But, none of the characters ever achieve their dreams. * Nature Steinbeck shows that nature is a beautiful and peaceful place, but threatened by the actions of men. The beginning of the story sets this pattern, as the creatures at the pool are disturbed by George and Lennies approach. The ranch and its buildings, being created by men, are in contrast with the natural world. For example the bunkhouse is quite bare and stark. Even more unnatural is that Candy and Crooks are either deformed in appearance. Contrasted to these two characters is Lennie, who almost seems a part of the natural world as he is described in animal terms. In fact, one of Lennies dreams is to go and live by himself in a cave. Maybe this would be the only way in which the natural world of Lennie would not come into conflict with the world of men.. Steinbeck referred to this story as a play/novel, and we are shown how closely it does resemble a play. Each section or chapter is set in a clearly defined place like a scene in a play. The beginning of each section contains detailed description, like stage directions in a play, while the rest of each section is mostly dialogue. This may seem like a rather unreal way to write a novel, but Steinbeck does it so well that we do not notice. A noticeable feature of the language of the novel is what a critic might call economy. One result of this is that almost every sentence is important in one way or another, either in developing a character, moving the plot forward or hinting at action still to come. Steinbeck has skilfully created a number of parallel events into the story. Candy and his dog provide a parallel to George and Lennie. And also, when Lennie kills Curleys wife, it echoes his earlier killing of the puppy. There are many such echoes and parallels in the book. Steinbeck has also shown us the way in which capturing the spoken language of the characters gives a better feel to the story. Most of them are uneducated, and this shows through in their use of broken and slang language. Even their uneducated pronunciation has been shown Steinbeck also uses colours and sounds to great effect. For example, Curleys wife is associated with red, symbolic of danger or passion perhaps. A further strong association is that of Lennie with animals. At various times he is described as a bull, a bear and a dog. Even when not directly compared with an animal, he is described in animal terms. For example, his hand is a paw. This is particularly appropriate for Lennie, as he usually acts in the simple, natural way of an animal. Soledad is a real place in California and its name can mean loneliness or a lonely place. A cue to one of the major themes of the novel right at the beginning.

It is a French word that could also mean or pertain to a situation that cannot progress at all due to misunderstanding between the parties concerned. One example of this situation would be the discourse between the employer and the employees regarding their Collective Bargaining Agreement. Labor history could prove that several negotiations of this kind has encountered several deadlocks hence, has become an impasse. It is basically due to the fact that at some points, the two parties concerned are not willing to sacrifice or give in to the conditions of the other. This scenario is very dangerous for it may result to boycott, demonstrations or protests as far as the employees are concerned. And on the other hand, it may result to retrenchment, as far as the employer is concerned. For there are two opposing interests, there would also be equal reactions to the issue concerned. As for me, if I would be the one to resolve this issue, I would find common grounds between the employees and the employer where each of them would benefit with the resolution of their problem. It may be in the form of an increase in the wages for this has been the long concern of workers around the world, and I believe is more just for both of the parties concerned. Case Study: Methanol Poisoning of a Child Introduction In this PBL, we observe a case of methanol poisoning in a child. We will first define the unfamiliar term of tertiary care centre and proceed to understand acid base homeostasis in the body, the basis for methanol poisoning. Then we will look at how ethanol and methanol are metabolised in the body followed by how methanol poisoning actually works. We will also discuss how the osmolal gap is obtained using osmolality and osmolarity and lastly child abuse. Unfamiliar term: Tertiary care centre A tertiary care centre is where a patient goes to when primary and secondary care have not been able to adequately treat the patient. Tertiary care centres are equipped with highly trained staff and highly dedicated medical equipment to cater to complex treatments or procedures as required by the patient. An example of a tertiary care centre would be the colorectal unit at The Royal London (1). Amareen was transferred to a tertiary care centre to receive more suitable care mainly due to the fact that she was so young and suffering from methanol poisoning. 2. Overview of acid base homeostasis in the body Acid base homeostasis is the regulation of hydrogen ions. The higher the concentration of hydrogen ions, the lower the pH and vice versa. Acidic solutions have a high pH whereas alkaline solutions have a lower pH. The normal pH in the body is in the range of 7.35-7.45. A pH lower than 7.35 results in acidosis whereas a pH higher than 7.45 results in alkalosis. Acid base balance has its basis in the Henderson-Hasselbalch equation shown in Figure 2. If we rearrange the equation, we see that bicarbonate and carbon dioxide directly affects the acid base balance. Figure 2: Henderson-Hasselbalch Equation (3) There are three main ways in which the body controls the acid base balance. These three systems usually work together. Firstly, there are physiologic buffers, each of which consist of a weak acid and its base salt or a weak base and its base acid. Physiologic buffers react immediately within seconds to the change in pH in the body. These buffer systems occur in both intra and extracellular parts of the cells. The main buffering systems for physiologic buffers are extracellular bicarbonate-carbonic acid buffering system, intracellular protein buffers and phosphate buffers in the bone. An overview of the physiologic buffer system is shown below in Figure 1. Figure 1: Physiologic buffer systems (2) If physiologic buffers are not enough to return the pH back to its normal value, pulmonary compensation can take place in the lungs. This works by eliminating or retaining carbon dioxide. Increased ventilation effort (hyperventilation) and decreased ventilation effort (hypoventilation) are the two ways pulmonary compensation works. The changes in pulmonary compensation is rapid within a few minutes. The final compensatory mechanism, renal compensation in the kidney, starts when the previous two mechanisms have failed to regulate the pH level in the body. The kidneys maintain balance by excreting or conserving bicarbonate and hydrogen ions in the body. However, this compensatory mechanism is a long term regulator and takes longer, usually a few hours, to respond to a change in acid base balance. The normal arterial blood gas values for partial pressure of carbon dioxide is 35-45 mmHg/ 4.7 kPa-6.0 kPa and bicarbonate concentration is 22-26 mmol/L. A change in the partial pressure of carbon dioxide or bicarbonate concentration from normal levels results in respiratory or metabolic disorders respectively. This together with acidosis or alkalosis determined by the pH results in 4 main disorders arising. Figure 3 below shows the 4 different disorders and their respective compensatory mechanisms. Figure 3: Diagnosis of the four main acid base disorders and respective compensation mechanisms (4). In Amareen’s case, metabolic acidosis occurs. The major cause is the high production of formic acid which is not excreted quickly from the body. This has caused a decrease in the bicarbonate concentration due to H+ combining with bicarbonate. By the law of mass action using the Henderson- Hasselbalch equation, there is extra CO2 generated thus Amareen shows respiratory compensation via tachypneic breathing whereby the peripheral chemo receptors in the lungs are stimulated which stimulates the alveoli to try to exhale the excess CO2. The exhalation of CO2 would in most cases be enough to correct the metabolic acidosis but in this case as methanol was ingested, external treatment was mandatory to save Amareen’s life. 3. How methanol and ethanol are metabolised in the body Ethanol metabolism Ethanol, commonly known as drinking alcohol, is metabolised primarily by alcohol dehydrogenase in the liver. Once ingested, ethanol is quickly absorbed by the gastrointestinal tract and small intestines with concentrations reaching at maximum level at 20-60 minutes (5). Ethanol is metabolised to acetaldehyde by alcohol dehydrogenase and then to acetate in the mitochondria via aldehyde dehydrogenase. Acetate is then metabolised to Acetyl CoA and subsequently to CO2 and H2O by the Krebs cycle in the mitochondria. Ethanol can also be metabolised by two other pathways: by cytochrome P450 2E1 (CYP2E1) in microsomes of the endoplasmic reticulum when there is a high ethanol consumption and by catalase in peroxisomes (6). The three ways ethanol is metabolised are illustrated in Figure 4 below. Figure 4: Oxidative pathways of ethanol metabolism in the body (7) Methanol metabolism Methanol, commonly known as wood alcohol, can be absorbed by the body via inhalation, ingestion and skin exposure (8). In this specific case, Amareen ingests methanol. When methanol is ingested, it is quickly absorbed by the gastrointestinal tract with concentration reaching a maximum level at 30-90 minutes after ingestion (8). Methanol is primarily metabolised in the liver. In the first step, methanol is metabolised to formaldehyde by alcohol dehydrogenase. Then formaldehyde is subsequently metabolised to formic acid by formaldehyde dehydrogenase. Formic acid is finally metabolised into carbon dioxide and water in the presence of tetrahydrofolate. This final step proceeds very slowly and hence there is an accumulation of formic acid in the body which is the chief cause of methanol poisoning and will be discussed in the next section. Methanol metabolism is illustrated in Figure 5 below. Figure 5: Metabolism of methanol (9) 4. How does methanol poisoning work Symptoms and Causes The main characteristics of methanol poisoning are metabolic acidosis and ocular damage. Formic acid, the metabolite of methanol and not methanol itself is considered to be toxic. The severity of the toxicity correlates with the degree of metabolic acidosis rather than concentration of methanol. (10, 11) The accumulation of formic acid in the body has many detrimental effects if left untreated. The effects of methanol poisoning can be grouped into different phases (12). The phases are described in Table 1 below. Table 1: Different phases of methanol poisoning Formic acid has been shown to inhibit cytochrome C oxidase activity in mitochondria (14) which is similar to the action of cyanide, hydrogen sulphate and carbon monoxide (15). Cytochrome C oxidase is the last enzyme in the electron transport chain of the mitochondria which results in the synthesis of ATP (16). Thus, by inhibiting cytochrome C oxidase, there would be significant reduction in the synthesis of ATP resulting in cell hypoxia leading to cell injury and death (17, 18). The amount of formic acid in the blood is proportional to the increase in the anion gap which measures the contribution of unmeasured anions to acidosis by using the formula [Na+] + [K+] – [Cl-] [HCO3-] (19). A high anion gap of 20mmol/dL was observed in the arterial blood gas of Amareen when she was transferred to the tertiary care centre. A high anion gap indicates the loss of bicarbonate ions without concurrent loss in chloride ions. Thus, a low serum bicarbonate level is a reliable indicator of the severity of methanol poisoning. Other causes of a high anion gap are diabetes keto acidosis, lactic acidosis, ethylene glycol and salycilate. Diagnosis Diagnosis for Amareen was relatively easy as a history was available from her parents stating that she had ingested methanol. This allowed doctors to treat Amareen quickly and correctly to prevent blindness or even death. If a history is unavailable, a test for the osmolal gap (refer to on how osmolal gap is derived) is very useful. A high osmolal gap (>10 mOsm/kg H20) indicates the presence of significant amounts of low molecular weight substances such as methanol. When methanol is metabolised, the osmolal gap returns to the normal and the anion gap increases due to formic acid formation which causes bicarbonate ions to decrease via the Henderson Hasselbach equation. A high serum methanol concentration of 35 mg/dL (> 0mg/dL), low serum bicarbonate level of 18mmol/L, low pH of 7.32 and a high anion gap of 20mmol/dL confirms Amareen’s diagnosis of metabolic acidosis cause by methanol poisoning. Treatments In this case, Amareen is treated with an ethanol drip. This is because like methanol, ethanol uses alcohol dehydrogenase as its first stage of metabolism and that ethanol has a higher affinity for alcohol dehydrogenase than methanol in the ratio of 20:1 (8). Therefore when ethanol enters the bloodstream, they will competitively bind to alcohol dehydrogenase thus inhibiting the formation of formic acid. In a clinical setting, a target level of 100-150 mg/dl is used to saturate alcohol dehydrogenase with ethanol (20). However, ethanol can be a challenge to administer due to irregular rate of metabolism making a steady target level difficult to maintain and it can also cause intoxication (20). Amareen later receives fomepizole treatment after the ethanol drip was not so effective. This is a better treatment because fomepizole has an even higher affinity for alcohol dehydrogenase than methanol in the ratio of 8000:1 (21). This prevents methanol from being metabolised. The advantages of fomepizole are the ease of administration, long duration of effect and that it doesn’t cause intoxication (22). However, fomepizole is very expensive and is less widely available (23). Due to both ethanol and fomepizole being ineffective in reducing the serum concentration of methanol in the body, haemodialysis was started. Haemodialysis is the most effective way to remove methanol and formic acid from the body (24). This works by passing blood from the body through a dialysis machine that contains a series of membranes to filter out unwanted substances and replenish essential minerals to the blood and then pumping blood back to into the body. The reason why haemodialysis was not immediately administered was probably due to it requiring a neck line which is very invasive and can result in multiple complications for Amareen who is only 5 years old. 5. Osmolarity and Osmolality Osmolality refers to the osmolar concentration of plasma per kilogram of solvent. Osmolality is measured using osmometers. Osmolarity on the other hand refers to the osmolar concentration of plasma per litre of solution. This value is calculated using a set formula from measured concentrations of Na+, K+, glucose and urea. The equation is 2[Na+] + 2[K+] + Glucose + Urea (all in mmol/L). Using osmolality and osmolarity, the osmolal gap can be calculated which is the difference between the actual osmolality and the calculated osmolarity which normally lies in the range of 8-10 mOsm/kg (25). 6. Child Abuse There are four main categories of child abuse (26). Physical abuse which involves bodily harm for example bruises, burns and fractures. Emotional abuse that involves persistent emotional ill-treatment or neglect causing adverse effects on the childs emotional development. Sexual abuse by forcing a child to perform sexual activity. This includes ‘non-contact’ sexual activities such as producing child pornography. Lastly, negligence which is the failure of carers to provide the basic physical and psychological needs as well as supervision from harm to the child which results in an adverse effect on the child’s health and development. The New Federalist Party Part I As the sole member of the New Federalist party, it is with great honors that I now present to you the very first New Federalist platform. PREAMBLE The growing dissension between the two major political parties today has drawn them away from the public's views. It has been determined that the citizens of the United States cannot get what they want from the current major parties. Because of this, a total reconstruction of the current political structure is in dire need. In response to this need, the New Federalist party has been formed. The name "New Federalist" has been chosen to express the party's foremost concern. This is to restructure the government into the form that the framers of the Constitution meant for it to be in. The basis of this restructuring comes from the 10th amendment and articles of the Constitution. As you know, every major political party needs a symbol. After careful consideration, the mythological hydra has been selected. I know what your thinking, but it is effective in serving two main purposes. The first purpose is that it is an ideal representation of the new structure of government that will be implemented by the New Federalist party. The two heads represent the two governing bodies, the federal government and the state governments. Both "heads" do nothing more than serve or govern over the body, which represents the citizens of the United States of America. The second purpose of the hydra is to shift the focus from the representative party to the individual members of the party. A political party should, like the hydra, should be nothing more than a myth. Votes in an election should not be cast for a party but for the individual candidates. The New Federalist party will be nothing more than a collection of like-minded people seeking to better our great country. In the following pages I have set forth the basic principles and various policy stands of the New Federalist party. STATEMENT OF PRINCIPLES AND POLICIES PRINCIPLES Federalism: "A political system in which ultimate authority is shared between a central government and state or regional governments."1 The first and foremost principality addresses the power of the federal and state governments. The framers of the Constitution never meant for the federal government to grow to today's tremendous size. The 10th amendment states that "The powers not delegated to the United States by the Constitution, nor prohibited by it to the states, are reserved to the states respectively, or to the people."2 This means that everything not set forth in the Constitution as being regulated by the federal government will go

Nuclear Weapons and Foreign Policy is a good example of the general mindset during the Cold War. Kissinger explains the nuclear dilemma and how much the nuclear age differs from past war problems. He also goes into depths about the problems with American foreign policy and offers several opinions on how it can be changed. At certain times, the book begins to read more as a Kissinger manual on how to fix foreign policy and becomes less of an analysis of nuclear weapons and foreign policy. This is most likely attributed to his positions as National Security Advisor and Secretary of State. His central thesis was that the threat of nuclear annihilation was nearly useless as an instrument of foreign policy because it lacked credibility. Instead, he offers that the United States focuses on improving its ability to fight limited wars, and accept that total victory in said wars were not crucial. He spends a great deal of time arguing the benefits of limited warfare, the problems nuclear weap ons bring into military operations, and the politics surrounding both. All said points were analyzed by Kissinger very well; however he fails to address the problem of arms control and the arms race in general. The main argument Kissinger addresses is that limited war was the only avenue of escape from nuclear stalemate with the Soviet Union. He argues that the United States tradition of absolute victory in war had led them into a dead end of massive retaliation and the threat of all out nuclear war was used to block every move by the Soviets. According to Kissinger, nuclear weapons coincided with the concept of total war. This leads to situations where the will to fight is crippled. He advocates a return to limited warfare similar to that of the Nap... ...ome above the military’s quickness to respond. During war militaries do not have the ability to define the difference between tactical victories and political goals. In a limited nuclear war, it is the political element which decides the length and breath of the military effort. Such was the nature of the task Henry Kissinger undertook during the heyday of the Cold War. Overall Henry Kissinger offered explanation to the problems foreign policy faced because of nuclear weapons during the Cold War. He believed that deterrence and limited warfare was the best solution to these problems. Today it would not seem like as big of deal, but at the time this was a very controversial viewpoint. I argue that Kissinger could have touched more on the arms race and other threatening regions, such as China. All in all, I enjoyed his viewpoint on a very conflicted time in history.
